Prior Authorization for Power Operated Vehicles and Orthoses

Effective April 13, 2022, Prior Authorization (PA) will be required nationwide for the following HCPCS codes for Power Operated Vehicles: 

  • K0800
  • K0801
  • K0802
  • K0806
  • K0807
  • K0808

Effective April 13, 2022, PA will be required for the following orthoses HCPCS codes in the states of New York, Florida, Illinois and California: 

  • Spinal Orthoses: L0648 and L0650
  • Knee Orthoses: L1832, L1833 and L1851

A PA will need to be on file prior to providing these items. 

The DME MACs will begin accepting PA requests for the Power Operated Vehicles and Orthoses HCPCS codes on March 30, 2022 for dates of delivery on or after April 13, 2022. For additional information, view the applicable webpages below for your jurisdiction.
